HOTSPOTTING IN NEW ORLEANS: DELIVERING SUSTAINABLE HEALTHCARE

Fewer than 1% of hospital patients are responsible for up to 23% of healthcare costs. In "hotspotting" projects around the nation, data mining identifies these super-utilizer patients, allowing interprofessional teams to critically evaluate these patients' unique situations to identify simple interventions that can dramatically reduce healthcare costs and promote sustainable healthcare delivery. In Fall 2014, two XULA Pharmacy students joined an interprofessional student team to dramatically impact the lives of 4 New Orleans patients. This grant proposes to continue this important Service-Learning work with a second cohort of interprofessional student-patient teams. Grant funds would be utilized to support establishing an elective course for curricular credit open to students of Xavier College of Pharmacy, LSU Health Sciences Center, and Tulane University.

This project seeks to help students: develop skills necessary for both patient communication and interprofessional communication and collaboration, develop empathy and understanding for patients with unmet medical needs, and develop project management, problem solving, and leadership skills.

Students would identify patients with high-utilizer status, and pair with another interprofessional team member to interview and work closely with their patient to reduce their excessive healthcare system utilization. Grant funds would be used to support these patients' needs and for travel assistance for student teams to present work at national conferences.
